PATCH TEST: I understand a patch test is recommended at least 48 hours before any color service if I have not had this service performed by this stylist before.

CHEMICAL HISTORY: I have disclosed all recent chemical services (color, relaxer, henna, etc.) and any medications affecting hair.

RESULTS: Hair color outcomes can vary based on starting condition, prior chemicals, and porosity. The stylist will work to achieve the requested outcome but does not guarantee specific results.

I release the stylist and goldenhour from liability for outcomes that are within the normal scope of professional cosmetology practice.
